This is challenging from a technical standpoint, because it results in a really wide form. Folks with 16:9 monitors and small resolution might be okay, but folks with older monitors (or on tablets/phones) will have to scroll horizontally and vertically. (That's a bad design practice.)

I'll have to brainstorm other ways to approach this.
